The printed form from 1950 that you found sounds very interesting, as it has required much interesting information.  &lt;br /&gt;

I am wondering if the Form shows the name of the place that it originated from, or where it needed to be returned to once it was completed?  Perhaps there is a wealth of information on these forms if they still exist somewhere in Poland and can be accessed.  &lt;br /&gt;
Would it be possible for you to scan the form so that we could all see it?&lt;br /&gt;

                                      End of July in Warsaw I found a printed form from 1950, the survey where everybody had to answer loads of questions, what was he/she doing before the war, during the WWII, after the WWII, what were doing his siblings and parents, and children, did he/she had any family abroad, what was his origin (in the political communist sense: working-class or worse), and biography.</description>
