Al,

I presume that this is the record that you wanted a confirmation of the first name of the person baptized. The name is Joannes /Baptista/ = Jan Chrzciciel = John the Baptist. In France it was/is not uncommon for a man to be named Jean Baptiste and to use both names. In Poland it was unlikely that he would have used the combination name. He most likely would just have been known as Jan. It seems that the priest was just making the name of the saint clear since there are quite a few Saint Johns.

Gilberto provided you with most of the important info in the record but I'll transcribe the Latin for you and fill in a few more details.

Dave

C. 1: # of Baptism for the year: 60
C. 2: Date of Birth: May 20
C. 3: Date of Baptism: May 22
C. 4: House #295
C. 5: Name of Person Baptized: Joannes /Baptista/ = cf. above
C. 6-10: Religion (Catholic); Gender: Male; Legitimate
C. 11: Father: Josephus Janicki filius legitimus Sebastiani et Catharinae n(atae) Hec hort(ulani) = Jozef Janicki, the legitimate son of Sebastian and Katarzyna born Hec, gardeners (i.e. owned a cottage and enough land for a garden but not fields for crops)
C. 12: Mother: Magdalena filia legitima Joannis Gatarski et Mariannae n(atae) Glowacki hort(ulani) = Magdalena, the legitimate daughter of Jan Gatarski and Maryanna born Glowacki, gardeners (cf. above)
C. 13: Sponsors: Laurentius Botch(?) hort(ulanus); Salomea uxor Antonii Lecki (I read the first letter of the surname as L with the diacritical mark) = Wawrzyniec Botch(?), a gardener; Solomea, the wife of Antoni Lecki
Notation in C. 6-10: Ob(stetri)x n(on) ex(aminata) Marianna Cempruch = The not examined midwife (was) Maryanna Cempruch (Records from the Austrian Partition list midwives as examinata (examined) or approbata (approved) or the negative of those categories. It appears that there was some sort of system in place to "license" midwives but it also appears that unlicensed midwives also practiced their trade/art.
Notation in C. 11-13: Baptisavi ego Josephus Fyda coop(erator) = I, Jozef Fyda, the assistant pastor, baptized (him).
