Here is the Pleśniak surname distribution map: http://www.moikrewni.pl/mapa/kompletny/ple%25C5%259Bniak.html (indeed it is very popular in Rzeszów and Błażowa area).

The birth records for years 1846-1904 from the parish of Błażowa are kept in the State archives in Rzeszów.
