Hi Joy,

The attachment from the Skorowidz… shows that the R.C. parish for Opacionka is in Brzostek. The church is under the title of the Exaltation of the Holy Cross. A picture of the church, which was constructed from 1812 through 1818, and the contact info can be found at http://www.diecezja.rzeszow.pl/2015/04/brzostek-2/

You might check Family Search & the Polish Archives for any records available online before contacting the parish directly, although a quick check of the archives lists vital records for the years 1890-1910 with nothing online.
