This is what I managed to find so far:
Surname Szostak was noted for the first time in 1406 and it can be derived directly from the noun szostak / szóstak, (old Polish coin equal to six - sześć - grosz).

Surname Przybyłowicz can be interpreted in two ways. A form of surmane Przybył/Przybyła - comes from the "who arrived". Surname Przybyła was noted in Poland for the first time in 1486. Surname Przybyłowicz was recorded in Poland for the first time in 1648.

Wieczorek surname comes from the noun evening and could originally mean someone born at this time of the day. K. Rymut (Surnames of Poles, Wrocław 1991, p. 289) states that this form was recorded in Poland in 1435.

Patok - this surname comes from the noun patok/potok, meaning: stream. Surname Patok was recorded in Poland for the first time in 1401.

Korzeniewski - surname created from the name of the town or village of Korzeniew, Korzeniewo or similar. Also could be created from the noun "root" = korzeń.
