I can order a copy of the document from the State Archives for you or You can do it by writing at this email address: sekretariat@agad.gov.pl where You should indicate what are You looking for and for what purposes (genealogical or other) and after You order it by email You will have to wait for the answer back message from the Archives, sometimes it takes 2 weeks.

Hi,
