Urban from given name Urban appearing in Poland from 13th century, of Latin origin from 'urbanus' "urban, inhabitant of a city".

Slawinski or, with Polish letters, Sławiński from the town of Sławno (a few villages). For the first time the surname appears in written sources in 1397.

Szach-; Szachowicz from szach "shah", or szachy "chess", or in Jewish names siftey, kohen "lips of the priest", pen name of a prominent Vilnius rabbi.

Guz-; (most probably) Guziejewski from guz "bulge, bump".
