According to www.herby.com.pl there is Karp and Karpel. No Karpet neither Karpeł, though there is Karpeta.
Karpiński. Feminine form Karpińska.
I don't know the feminine form for Karp. Maybe Karpowa, not sure.

Karp
Karpet
