Thank you so much! I knew you could help me. This makes a lot more sense now in terms of interpreting this document. Now, I have a bigger mystery as I try to reconcile this document with other documents I have for Hinde in the US. I am trying to determine if the Hinde in this document is the same Hinde in the US documents or is it just an odd coincidence that they would have the exact same name and the exact same birthdate. The problem is the dates don’t line up. &lt;br /&gt;
&lt;br /&gt;
I am waiting for a marriage document from Massachusetts that I hope will settle it by including parents names. &lt;br /&gt;

If not, I may be back asking for your help again!&lt;br /&gt;

Cynthia, there are two places where there is a date stamp on the first page. In the upper left-hand corner you see &lt;span style=&quot;font-weight: bold&quot;&gt;3 LUT. 1935&lt;/span&gt; but with the long extraneous mark on the page, it could just as easily be &lt;span style=&quot;font-weight: bold&quot;&gt;13 LUT. 1935&lt;/span&gt; which would match the date of Hinde's signature on the second page, February 3, 1935. The other date stamp in the lower right-hand corner is the one you asked about, and while the stamp itself is too faint to read in full, you can see that it includes &lt;span style=&quot;font-weight: bold&quot;&gt;9 GRUD. 1938&lt;/span&gt; meaning December 9, 1938, so it is interesting that whatever the function of that stamp was, it was happening so many years later. If you have pulled this image of the application form from someplace where you have full access and you can scroll through other people's applications, this stamp will likely appear on their application forms too and maybe one of those will be more legible. The format III/22/497/31 (not sure about that 31) reminds me a bit of codes used by USCs (civil registry offices). &lt;br /&gt;

I agree with the place names as Barb has written them. Regarding the cross-outs on the first page, it was simply an error since she was single (as shown on the second page) and so she would not have needed to fill in the part of the first page that concerned marriage.&lt;br /&gt;
&lt;br /&gt;
I will add one small detail. When you labelled the images of the two pages you called it a &amp;quot;passport.&amp;quot; It appears to me to be an application for a &lt;span style=&quot;font-weight: bold&quot;&gt;dowód osobisty&lt;/span&gt; (identity document) which would then have been issued by the local authorities in Stanisławów. It is not the identity document itself. Barb, please let me know if I've got that right or wrong.&lt;br /&gt;

1.The first part indicates where the birth register was issued. There I can read that the birth register was issued by the Jewish civil registry office in Rożniatów on July 5, 1934.&lt;br /&gt;
The second part indicates where the marriage register was issued.&lt;br /&gt;
Since this document concerns a birth, it appears that the information was written in the wrong place and was therefore later crossed out.&lt;br /&gt;
&lt;br /&gt;
2.There appears to be a stamp with an added registration number. I cannot help with that. It is very unclear. Maybe it refers to the date of registration in Stanisławów in 1931?&lt;br /&gt;
&lt;br /&gt;
3.She was born in Broszniów, but the document was issued by the Jewish civil registry office in Rożniatów. That is why you see two different names for places.&lt;br /&gt;
&lt;br /&gt;
4.The place is called Żydaczów.&lt;br /&gt;
&lt;br /&gt;
Perhaps someone else can provide more information.&lt;br /&gt;

I managed to translate most of this document, but I need some help with a couple of sections:&lt;br /&gt;
&lt;br /&gt;
1. On page 1: bottom section with handwritten answers to questions about the location of her vital records. I can't make out the handwritten answers. Some of it is crossed out and I don't know what that means. Can someone help me translate this section please?&lt;br /&gt;
&lt;br /&gt;
2. On page 1: At bottom right hand corner, there is a handwritten clerical note of some kind. Can anyone tell me what the abbreviations may mean?&lt;br /&gt;
&lt;br /&gt;
3. On page 1: It seems that her first answer to location of her birth (top of page) does not match the later section at bottom of page asking for location of her birth record. Am I seeing this correctly? Are there two different answers?&lt;br /&gt;
&lt;br /&gt;
4. On page 2: the section asking for names and birth place of her parents. Can someone make out the handwritten answer for birthplace of parents?&lt;br /&gt;
