I disagree as death certificates have very accurate information pertaining to the death of that person. If your are referring to "parents information" on the death certificate this is usually provided by a family member. Blame them. This info can be lacking based on the informants knowledge usually from a close family member or even a neighbor or friend in extreme cases.

I would check for Barbara's passenger ship record for the name of the place she came from in Poland. Also her Naturalization papers may list the town. Check the census records for the approximate year filed.

This is the last name that is mentioned on my mother's death certificate as her mother's last name. Death certificates are really bad in the US as a source for accurate information. My grandmothers name was Barbara Kotyla (December 17, 1885). I am told she is from a farm area near Kracow. I am unable to find out where. She married Matthew Harasiuk( May 26, 1879) and probably from the area around Szwedy. There was a brother named Kasimir. Any information would be greatly appreciated.
Thanks
