Hi,

If you hover your cursor over the middle icon in the “Uwagi” column you will see where the document is housed—Panstwowe Archiwum Historyczne w Wilnie. In order to obtain an image of the record you should contact the archive. Note that the archive is in Lithuania and not in Poland.
