I would be very grateful for any advice on how to obtain a Polish death certificate&lt;br /&gt;

I am trying to find details about one of the relative of my Mother in Law. We have scant information. Her name was Hedwig Poloczek and we know she died during WW2 in Lubliniec Poland, from TB contracted from a patient at a Physciatric/Mental hospital she was working at. We are reasonably confident she was around 50-60yrs old when she died and her parents were Constantin Poloczek and Florentyna Ziaja.&lt;br /&gt;
&lt;br /&gt;
We think she was born in Tworog or surrounding areas but have been unable to find a record of birth in LDS records and LDS death records for Lubliniec do not cover this period.&lt;br /&gt;
&lt;br /&gt;
Are there equivalents to UK Registry Offices or the GRO in Poland that we can write/email and ask to search for her deatth record? As if we are able to find this then we are hoping this will open more doors to finding more information about her.&lt;br /&gt;

Here goes the addresse of the Civil Offices of the places you mentioned. I am not sure USC will perform any genealogical research. Records less than 100 years old are usually provided only to imediate family members. YOu could also try the State Archives to search for Konstanty and Florentyna marriage record. However, since there is two Tworog I don't know if yours is the one under AP Katowice (oddzial Gliwice) or AP Czestochowa area.&lt;br /&gt;
