I also found that he arrived in NY on June 3, 1893 and not on the date on the application.

I have attached the snapshots of Syzmon Kwak's passport application that I found online. I do not have the original document. The translation of his mother's affidavit is on the last photo.

I am trying to find out more information on my mother's side of the family. I have been very successful with their time here in the US but am now trying to gather some information about them in Poland.
