Hello!

I have stumbled across my 3rd great grandfather's (Tomasz Zawol) death record from 1879. I'm hoping it states the names of his parents; specifically, I'm hoping this includes his mother's maiden name and of course, it's in Cyrillic Russian limiting my ability to read it. There's no rush, but if someone has time, can you translate this for me? I greatly appreciate the help.

Record 1

Thank you,

Connie

Could you please provide the translation of the underlined words? Most of them are names and I want to get another opinion on how they are spelled.
Thanks,
