This document is written in polish, and it says more or less:

December 9th, 1831, number 809.

Kolomyia Circular Office asks if, according to the attached documents, Tomasz Ostrowski can be considered a nobleman.

In the year of 1812, on October 3rd, at 9 a.m., in front of Us, the Civil Registrar for the commune of Kuczbork, located in the county of Mława, in the department of Płock, appeared: Krystian Tank, farmer, 38 years old and Michał Roznoch, farmer, 40 years old, both living in Olszewko and they declared Us that on the 2nd day of the current month and year, at 8 a.m., died Michał, 7 years old, son of Jan Freychel, farmer, 50 years old and Katarzyna nee Wadyna, who live in Olszewko in the house number 8.
This death record was read to the declarants and was signed by Us, the Civil Registrar, because the declarants were illiterate.

Civil Registrar, Jakub Aleksandrowicz.
