It happened in Myszyniec, on January 5th/18th, 1915, at 1 p.m.
Appeared: Józef Abramczyk, 44 years old and Ignacy Ałaj, 50 years old, farmers from the village of Myszyniec Stary and they declared that yesterday, at 12 o'clock at noon, died in the village of Myszyniec Stary: Anna Wachowska, 37 years old, daughter of Piotr and Marianna, the spouses Domian, farmer, who was born and lived in the village of Myszyniec Stary, who left her husband, Piotr Wachowski.
After convinced myself about the death of Anna Wachowska this act was read to the illiterate present and it was signed by us.

Administrator of the Myszyniec parish, serving as Civil Registrar, priest Fr. Karwacki.

It happened in the village of Cisów, on May 5th, 1895, at 12 o'clock at noon.
Appeared personally Franciszek Kałucki, a smith, living in the village of Widełki, 49 years old, in the presence of Jan Mąchocki, 66 years old and Tomasz Bernat, 44 years old, peasants living in the village of Widełki, and presented Us a female infant child, informing that the child was born today, in the village of Widełki, at 7 a.m., of his legal wife, Marianna nee Bukliowska, 46 years old.
At The Holy Baptism, held today by the priest Wincenty Majewski, the child was given the name: Stanisława, and the godparents were: Antoni Kaczmarczak and Józefa Biernat.
This act was read to the declarant and the witnesses, who were illiterate and it was signed by Us only.

Priest Wincenty Majewski, the administrator of the Cisów parish, serving as Civil Registrar.

On the left margin there is a note: 1. V. I guess it was written much later by some Civil Registrar, but he did not read the birth date correctly. He wrote May 1st, but it should be May 5th.
