Below there are sources of information about surnames we use while replying to your questions:
- Nazwiska Polaków (Surnames of Poles) by prof. Kazimierz Rymut, Krakow 1992.
- Nazwiska Polaków. Słownik historyczno-etymologiczny. (Surnames of Poles. Historical and Etymological Dictionary) by prof. Kazimierz Rymut, Warszawa 1999.
- Polish Surnames: Origins and Meanings by William F. Hoffman, Chicago, 1998.
- Spis Szlachty Polskiej (Register of Polish Nobility) compiled by Jerzy Sewer count Dunin-Borkowski and published in 1887 in Lwow.
- Database of surnames used in Poland compiled in 2002 by prof. K.Rymut - Dictionary of Surnames in Current Use in Poland.
- 1000 Najpopularniejszych Nazwisk w Polsce (1000 Most Popular Surnames in Poland) by Jarosław Maciej Zawadzki, Warszawa 2002.
- Jewish Family Names and Their Origins, by Heinrich Walter Guggenheimer, Eva H. Guggenheimer, New York 1992.

Nazwiska Polaków seems to be out of print. Couldn't find it even in Ebay.pl!

It appears from time to time on the Polish auction/ecommerce website Allegro.pl (equivalent of eBay in Poland). The two volume set starts from about 900 zlotys.
