After convinced myself about the death of Hersz Januszewski this act was read to the illiterate declarants and it was signed by us.

Appeared Maciej Ławniczak, 30 years old and Franciszek Bartecki, 35 years old, both farmers, living in the village of Augustynów and they declared that on the 14th/26th day of the current month and year, at 8 a.m., died in Augustynów: Wiktoria Kasznia, 6 years and 6 months old, daughter of Józef and Antonina nee Bartecka, the Kasznia spouses, farmers living in Augustynów.
After convinced myself about the death of Wiktoria Kasznia this act was read to the illiterate present and it was signed by Us.
