Good evening,

Could I have assistance in translating a document? I believe it is a 1913 certification of a birth or baptism. It belong to Jonas Wolis Who was born December 15, 1885. I am trying to learn the following.
The correct spelling of his name
The names of his mother and father
The name of the village or area where he camel from so I can research his roots in Europe. I have attached a photo of the certificate
Any help would be greatly appreciated. Thanks,
