The translations follow.

No.60 Oracz Moszek
It happened in Skarżysko-Kamienna, on August 26th, 1940.
In the presence of the undersigned Civil Registrar, the jew, appeared today Icek Ajzyk Oracz, a shoemaker, 37 years old, living in Skarżysko-Kamienna, in the presence of the witnesses Josek Mendel Waksberg, a tailor and Mendel Szyfman, a furniture upholsterer, both adult residents of the city of Skarżysko-Kamienna and presented Us a male infant child, who was born in Skarżysko-Kamienna, on November 15th, 1925, at 10 a.m., to his wife Sura Janklewicz, 39 years old. The born was given the name Moszek.
This act was delayed for family reasons, was accepted, and signed after being read.

The Civil Registrar: unreadable signature
Father: Icek Oracz
Witnesses: Mendel Waksberg and Mendel Szyfman

Decision of the Mayor of Skarżysko Kamienna of 18th March 2003
Or.III/8040/14/03 the surname was changed from Oracz to the surname ORYAN and the name from Moszek to the name MOSHE.
Skarżysko-Kamienna, on April 3rd, 2003
For the Civil Registry Office
unreadable signature
