Welcome to the forum!
This record is written in Latin and while I am not the best helper on this forum for Latin records (that would be Dave), I will try to assist you with the names. It would be easier to help you if I knew where this record was from because then I could look at what surnames are common in that parish. It can be helpful, when posting records, to provide a link to where you found the record if it was found online, or else an image of the whole page so it is easier to study how the writer formed their letters when writing.
The person being baptized is Anna Maria and the abbreviation in parentheses following her name is confirming for you that she was given two names. Beneath her name is the abbreviation "Ob" telling you who her midwife was, Maria with a surname that is a little hard to decipher. It could be Danecka, I'm not sure, and it's probably not important to you.
Anna Maria's father's name is written as Joannes, which in Polish is Jan and in English is John. You probably know the surname already. Does it begin with D? Derzanowski? Jan's mother's name was Antonina B. Anna Maria's mother's name looks like Paulina Pleczynska, the daughter of Kajetan Pleczynski and Frederika Krap.
I'd be happy to take a second look if you could give me more to work with.

Dear Sophia,
Thank you for responding to my question and providing me with the names on the document. Jan's last name is Berczoski aka Brosco. Unfortunately, I too, could not decipher the handwriting.
I did find document on the genteka site with Anna's parents listed. Attached is a photo of my findings. I did try a search on their site to locate more information but was unsuccessful reading the documents. Perhaps you can translate?
