It happened in the town of Szydłowiec, on May 24th/June 5th, 1892, at 10 a.m.
Appeared Szmul Chomentowski, 42 years old and Hersz Josek Krauze, 62 years old, a synagogue sextons living in the town of Szydłowiec and they declared that on May 23rd/June 4th of the current year, at 5 p.m., died Abram Jakub Szmulewicz, a tinsmith-master, 48 years old, who lived in the town of Szydłowiec, a son of Izrael Icek and Sura Etla nee Tandetnik. He was survived by his wife, Maria.
After convinced myself about the death of Abram Jakub Szmulewicz this act was read to the witnesses and due to the Krauze's illiteracy it was signed by Us and Chomentowski.

The signature of the mayor of the Szydłowiec town, serving as Civil Registrar.
