Appeared: Franciszek Czyż, 26 years old and Piotr Ciuk, 40 years old, both townsmans, farmers, living in Liw and they declared that yesterday, at 6 p.m. died Katarzyna Ciuk, 80 years old, townswoman, widow, daylabourer, daughter of parents unknown to the declarants, she lived in Liw and left her 4 (four) children: Andrzej, Piotr, Marianna and Franciszka.
After eye belief about Katarzyna Ciuk's death this act was read to the declarants of which the second is the deceased's son, it was signed by Us because the declarants are illiterate.

Vicar priest Jan Rogucki's signature on the behalf of parish's parson.

Hello everyone, I am new to this forum and I need your knowledge and help.
My sister died recently, and her colleagues found the notes in her desk drawer - which I attached it right now. We assume that these notes are hers because it was in her desk drawer, and also the hand-writing seem to be similar to hers as well. She loved Poland and always was writing in Polish even at home.

She was living in Poland and was doing her graduate programme while working part time. PLEASE PLEASE AND PLEASE help me to translate the notes. I beg you, please!
