The village of Zaczopki

It happened in the village of Pratulin, on June 7th/20th, 1909, at 6 p.m.
Appeared personally Damian Osipiuk, 50 years old, farmers from the village of Zaczopki, in the presence of Leon Chryciuk, 49 years old and Aleksy Łukaszuk, 44 years old, both farmers from the village of Zaczopki and presented Us a male infant child, informing that the child was born today in the village of Zaczopki, at 3 p.m., to his legal wife, Unisa nee Franczuk, 39 years old.
At The Holy Baptism, held today by Us, the child was given the name: Jakub, and the godparents were: Leon Chryciuk and Pelagia Kopaczuk.
This act was read to the declarant and the witnesses, all of them illiterate, and it was signed by Us only.
Priest's signature, the administrator of the Pratul parish, serving as Civil Registrar.
