Appeared Marcin Mazur, 42 years old, a farmer, living in Brok in the presence of Jan Aleks, 38 years old and Wojciech Szumski, 60 years old, both farmers living in Brok and presented Us a newborn female child, informing that the child was born in Brok on January 31st / February 12th., of the current year, at 5 a.m., to his legal wife, Katarzyna nee Okrasa, 37 years old.
At The Holy Baptism, held today by the priest Karol Milanowski, the vicar of the Brok parish, the child was given the name Marianna, and her godparents were: Jan Aleks and Franciszka Domasik.
This act was read to the declarant and the witnesses, who were illiterate, and it was signed by Us.
Priest Stawirej, the administrator of the Brok parish, serving as Civil Registrar.

On the left margin there is additional note: On April 1st/14th 1913, in the parish church in Brok, Marianna Mazur, a daughter of Marcin and Katarzyna nee Okrasa got married with Aleksander Szumski, a single man. Note signed by the priest Henryk Kuskowski.
