End of July in Warsaw I found a printed form from 1950, the survey where everybody had to answer loads of questions, what was he/she doing before the war, during the WWII, after the WWII, what were doing his siblings and parents, and children, did he/she had any family abroad, what was his origin (in the political communist sense: working-class or worse), and biography.

Siamon,

The printed form from 1950 that you found sounds very interesting, as it has required much interesting information.

I am wondering if the Form shows the name of the place that it originated from, or where it needed to be returned to once it was completed? Perhaps there is a wealth of information on these forms if they still exist somewhere in Poland and can be accessed.
Would it be possible for you to scan the form so that we could all see it?

Thank you for sharing your interesting "find" with us.

Mary Anne,

I typed from the original found in Warsaw the form from 1950, it is attached to the topic "1950, Ankieta Personalna"
http://polishorigins.com/forum/viewtopic.php?p=11287#11287

There are interesting comments on the origin of it.

Siamon,

It is unfair to quote without sources.

Elzbieta,

Thank you for the reference to your original post regarding this Form.

You did a lot of work re-typing it for us to see! Thank you!

I do hope that some day we will be able to find and see the original forms that were submitted by our family members. What a Treasure that will be..
