Post subject: Research in Poland-an interesting Form located
Siamon,
The printed form from 1950 that you found sounds very interesting, as it has required much interesting information.
I am wondering if the Form shows the name of the place that it originated from, or where it needed to be returned to once it was completed? Perhaps there is a wealth of information on these forms if they still exist somewhere in Poland and can be accessed.
Would it be possible for you to scan the form so that we could all see it?

I typed from the original found in Warsaw the form from 1950, it is attached to the topic "1950, Ankieta Personalna"
