Could someone please tell me where the Michael Kohola mentioned on this death records was born? I cannot make it out. Also, any help with the Kohola surname would also be much appreciated! My family now goes by the surname Hahula. Is there any relation between these two names or is this perhaps just a misspelling or a language barrier issue?

Hahula is Polish name.
Kohola does not exist in Poland. I guess a typo or bad deciphering, not to mention a mishmash between Latin and Cyrillic.

Elzbieta,

Is smolica or snolica a place in Poland? Or was it a place in Poland in the early 1900's? I suspect that his death certificate has snolica or smolica as his birthplace. However, an immigration record has smolice listed as their town.

Yes, Smolice is a place in Poland, 5 villages with the same name:
