The parish-priest of the romancatholic parish of Łomża informs that in the Civil Records of the said parish the following entry numer 279 from the year of 1864 was made:
It happened in the town of Łomża, on October 4th/16th, 1864, at 1 p.m.
Appeared Józef Modelski, a worker living in Kupiska, 22 years old, in the presence of Stanisław Dardziński, 43 years old and Paweł Zaczek, 50 years old, both farmers living in the said village and presented Us a newborn male child, who was born yesterday in Kupiska, at 10 p.m., to him and his wife, Julianna nee Jenart, 22 years old.
At The Holy Baptism, held today by the priest Włostowski, the child was given the name Józef and his godparents were Stanisław Dardziński and Katarzyna Jackowska.

I hereby declare that in the romancatholic parish of Wilków a three banns of marriage were announced, between:
Józef Modelski, a single man, a clerk living in the village of Wilków located in the Wilków parish and Zofia Franciszka Izabela Krauss, a miss living at her family in the village of Wilków located in the Wilków parish, on:
May 29th, June 5th and 12th/June 10th, 17th and 24th of the current year, that is 1894.
There were no objections for this marriage.
At the request of the above persons, I authorize the priest Bolesław Zimnicki, the vicar of the Church of the Nativity of the Blessed Virgin Mary in Warsaw to perform the religious marriage ceremony in the said church, between: Józef Modelski and Zofia Franciszka Izabela, of three names, Krauss and I also authorize the Civil Registrar of the said parish to draft the marriage act between the above said newlyweds and enter this act in the church book of the said parish, located in the city of Warsaw.
The village of Wilków, June 12th/24th, 1894.
