It happened in the town of Płock, on May 19th/31st, 1881, at 12 o'clock at noon.
Appeared Szmul Gedalia Hendel, a trader, 31 years old, living in the town of Płock, in the presence of the witnesses, Mosiek Buk, 62 years old and Berek Niatan, 36 years old, both the synagogue sextons living in Płock and presented Us a male child, who was born in the town of Płock on April 3rd/15th, 1880, to his legal wife, Maria Dwojra nee Taub, 32 years old.
At the circumcision rite the child was given the name Lipa.
The reason for the delay in drawing up the act was negligence.
This act was read to the declarant and the witnesses and it was signed by Us anf the present.
The signature of the Civil Registrar, illegible,
the signatures of the child's father and the witnesses.

Przerośl
It happened in Przerośl, on October 30th/November 11th, 1888, at 2 p.m.
Appeared Zuzanna Freze, a midwife, living in Przerośl, 49 years old, in the presence of Paweł Kułak, 44 years old and Jan Krynicki, 39 years old, the daylaborer living in Przerośl and presented Us a female infant child, who was born in Przerośl on October 27th/November 8th of the current year, at 10 p.m., and declared that the child was born to Wojciech Olszewski, 30 years old, who went abroad on February 15th/27th, 1888, who was not present ar drawing up this birth certificate because he went to America, and his legal wife, Tekla nee Boryszewska, 37 years old living in Przerośl.
At The Holy Baptism, held today, by the priest Kazimierz Łopieński, the child was given the name Leontyna and her godparents were: Stanisław Korotki, a son of a farmer from Przerośl and Michalina Olszewska.
This act was read to the declarant and the witnesses and it was signed by Us. The declarant and the witnesses were illiterate.
Priest Kazimierz Łopieński, the Przerośl parish-priest, serving as Civil Registrar.

Hi,
I can't read Russian but you might find the attached scan interesting, though with some mixed information. Both his marriages are there as well as the marriage of his son Stefan in 1919, who was born in Warsaw. The relevant 'skan' for Jan's marriage in 1925, also gives Warsaw as his address. You can d/l the skans from Geneteka, with the latter two being in Polish.

Hi Judy,
Jan Mamak was born ab. 1872 in Krakow (parish of St. Mikołaj). Son of unknown father. Since the mother's name is Marianna Mamak, it seems that he was born out of wedlock.

The child's name is Edmund, the father's name is Gustav Jeske as far as I know. The place is Gorki.

Appeared Gustaw Jesske, a peasant from Górki, 36 years old, in the presence of Wilhelm Gartwich, a peasant from Górki, 49 years old and Ludwik Kriger, a peasant from Górki, 42 years old and presented Us a male infant child, who was born in Górki on March 14th/27th of the current year, at 4 a.m., to his legal wife, Karolina nee Górke, 27 years old.
At The Holy Baptism, held today, the child was given the name Edmund and his godparents were: the first witness and Ernestyna Kriger, married.
This act was read to the declarant and the witnesses, who all were illiterate and it was signed by Us.
