My husband's grandfather has been doing research for decades. When we were down for a visit this summer, he mentioned he had found a few Polish relatives, and I was immediately interested. He pulled this document out and said he received it as a response, but hasn't found anyone to translate it for him. I was hoping you kind people could help! I've attached the picture below, but if you IM me, I could also email you the original resolution file.

In reference to the letter we have received concerning copies of vital records, we [the Registry Office in Mikolow] kindly inform you that we do not have any records containing the surname requested even though in our collections we have registers from the end of October 1875.
We recommend that you re-examine the surnames, dates and places of events, and then file a request for records. Please not that your future correspondence, if any, should be sent to the Polish Embassy in the USA, which will request the records on your behalf from the particular offices in Poland. Such a procedure applies in such cases, and any doubts can be clarified at the Embassy.

No cóż kochany, ładnie to wygląda mieć tylu potomków w naszym nazwisku, dobrze by było, żeby się z każdym spotkać, ale to jest niemożliwe, można sobie na papierku przeczytać, że tyle wśród nas i przed nami żyło z tym nazwiskiem. Gilberto, co do mnie, to ja i moja rodzina na razie wszyscy jakoś żyjemy, my starsi to wiesz – raz lepiej, drugi raz gorzej. Wiek swoje robi.
