If I may get some assistance regarding this birth document, I'll translate what I think I know and maybe get the blanks filled in

Hi,
Filling in the blanks would not be enough to provide you with an accurate translation so I felt that it would be better to post the translation of the entire entry. The abbreviations found in the entry specify that the parents were legally married and stand for the adjectives which were used to specify the status of peasants according to the feudal system used in the 18th Century Polish-Lithuanian Commonwealth, commonly referred to in Polish as Rzeczpostpolita Obojga Narodów or in Latin as Res Publica Utriusque Nationis, terms which emphasized the reciprocal guarantee of the rights of the two nations, the Kingdom of Poland and the Grand Duchy of Lithuania.

I, Wawrzyniec Gadomski, pastor of Niedźwiadna, baptized an infant by the name of Sylwester, the son of the legitimate marital union of the industrious* Jakub and Brygida Samełko; those lifting him up from the sacred font** were the upright*** Kazimierz Zaewski with the upright***Rozalia Powiadowska; those assisting*** were the upright**** Maciej Łatoska with Apolonia, the consort of the upright**** (illegible) Koz??kowski
Notes: *laboriosus/industrious: an adjective used to designate an individual as a peasant.
**levantes de Sacro Fonte/tose lifting him up from the Sacred Font: a circumlocution for sponsors aka godparents
***asistentes/those assisting: Catholic Church practice allowed for only two sponsors/godparents. When the parents of the child wished to include additional individuals those individuals, who had no official role in the rite were termed as “those assisting”.
