It happened in the village of Żabianka, on February 9th/22nd, 1915, at 9 a.m.
Appeared Andrzej Janiszek, 25 years old and Paweł Rutkowski, 42 years old, both farmers from the village of Białki Dolne and they declared that on February 7th/20th of the current year, at 5 a.m., died in the village of Białki Dolne Władysława Witosław, 1 month old, born in Białki Dolne, a daughter of the living Witosław spouses: Józef and Helena nee Kozdroj, workers.
After convinced myself about the death of Władysława Witosław this act was read to the illiterate declarants and it was signed by Us only.
The administrator of the Żabianka parish - priest's signature.
