Post subject: Researching Kwak Family in Poland
I am trying to find out more information on my mother's side of the family. I have been very successful with their time here in the US but am now trying to gather some information about them in Poland. My mother's maternal grandfather was Szymon Kwak who states on a US passport application, dated February 7, 1922, that he was born on October 28, 1863 in "Rogoznik Province Nowy Targ Little Poland". His father is noted as Bartlomiej Kwak, deceased.
It states that sailed from Poland to the US "about May 1903". He arrived in New York and resided in the Wilkes-Barra, Pennsylvania.
There is an affidavit attached, that was translated from Polish, from his mother, "Maryanna Kwak, age 90, residing in Ludzimerzu County Nowy Targ Malo Polska".
Also mentioned are her 2 other adult children. A son named Ludwik Kwak and a daughter named Wiktorya (Kwak) Czaja. Both of them still residing in Poland.
She was requesting the presence of Szymon because of her "declining age of 90", wishing to see her son again before she passed.
Would appreciate any information if someone know of them or who could direct me where to possible find additional information.

I have attached the snapshots of Syzmon Kwak's passport application that I found online. I do not have the original document. The translation of his mother's affidavit is on the last photo.
